Fresno Healthy Communities Access Partners (FHCAP) is a five year old nonprofit organization consisting of eleven health care and community organizations working together to improve access to health care for medically underserved communities in Fresno and the San Joaquin Valley.
Our twelve partners are: Central California Faculty Medical Group Central Valley Health Policy Institute Children’s Hospital Central California Clinics Sierra Vista Community Medical Centers Fresno/Madera Medical Society Fresno County Department of Public Health Fresno Metro Ministry Kaiser Permanente Saint Agnes Medical Center UCSF-Fresno, Medical Education Program Valley Health Team HCAP Vision Statement The Fresno Healthy Communities Access Partners is envisioned to serve as a foundation for greater community cooperation to meet health needs. Through coordination and collaboration, implementation and resource allocation decisions can benefit the health care of all Fresno County residents.

The Community We Serve
In Fresno County alone more than 162,000 individuals including 31,000 children were without health insurance coverage as of a 2005 CHIS study. In the entire San Joaquin Valley the number of uninsured reaches more than 550,000 individuals including 108,000 children. Many in this population are minorities.
